Obtained by activating Fatal Time 10 times, you will likely get this naturally while playing, keep attacking Wraiths to trigger Fatal Time.

Dodged a grab attack for the first time.
You need to press Cross at the right time to dodge an enemy when they try to grab you.
Avoiding Eye Contact
Dodged a leer attack for the first time.
Sometimes during battle or when opening doors, wraiths will leer at you. You need to dodge before your eyes meet. An easy way to achieve this is in Chapter 2 at the Minakami Village Kureha Shrine after getting the Right Jizo Key. When trying to leave the shrine, cancel the door-opening animation to dodge a leer attack.
